By submitting this application:
I give permission to Dallas CASA to inquire about my qualifications and/or character. I understand that this reference check may be done by phone or in writing or by electronic communication and may include present/past employers, volunteer organizations, personal references, agencies and the internet.
I give my permission for Dallas CASA to obtain information relating to my criminal history record. The criminal history record, as received from the reporting agencies, may include arrest and conviction data as well as plea bargains and deferred adjudications. I understand that this information will be used, in part, to determine my eligibility to serve as a Court Appointed Special Advocate with this organization.
I also understand that as long as I remain a volunteer advocate at Dallas CASA, the agency will repeat this criminal records check periodically. I understand that I will have an opportunity to review my criminal history report, and a procedure for clarification is available, if I dispute the record as received.
I, the undersigned, do, for myself, my heirs, executors and administrators, hereby remise, release and forever discharge and agree to indemnify Dallas CASA and each of their officers, directors, employees, actions, suits, liabilities, costs, debts, and sums of money, claims and demands from the investigation of my background in connection with my application to become a volunteer for Dallas CASA.
